Customising consent forms (consent form builder)

How to use the consent form builder to choose questions, wording, risks and branding for each hospital's consent and admission request form.

Open Consent Forms β†’ Edit Consent Forms to open the Consent form builder. It works the same way as the registration form builder: settings and questions on the left, and a live PDF preview on the right.

⚠️

Your changes won't appear on consent forms until you click Publish. The Publish button is at the top right of the builder (and again at the bottom). Save draft only saves your work in progress; new consent forms keep using the last published version until you publish.

ℹ️

You don't have to set anything up before consenting patients. Every hospital starts with the standard consent form. Use the builder only if your clinic wants to change it.

Templates and hospitals

A consent form template can be used by several hospitals, but each hospital can only use one template. This means you only build one form if several hospitals need exactly the same form, and you can make a separate one for a hospital with different requirements.

  1. Choose an existing template from Template, or click New template.
  1. Enter a Template name.
  1. Under Hospitals, tick the hospitals that should use this template. If a hospital is already using another template, ticking it here moves it to this template when you publish.

Form settings

  • Form title β€” the heading on the form.
  • Practice logo β€” upload a PNG or SVG, at least 400 pixels wide. A transparent background works best.
  • Accent colour β€” appears on the form and its PDF.

Choosing questions

The form is grouped into sections: Patient details, Procedure, Consent statements, Risks, Signatures, Hospital admission request, Pre-admission screening, and Contacts & referrals.

  • Set each question to Off, Optional or Required. Some questions, such as the patient's name, date of birth and contact number, are Locked because the form needs them.
  • Edit the Question wording and Help text to suit your clinic.
  • Turn the RAPT & discharge plan section on or off.
  • Use Add question to add your own custom question.

Custom consent clauses

Click Add clause under Custom consent clauses to add your own consent statements to the template.

Risks

  • Template-wide risks appear on the form at every hospital assigned to the template. Click Add risk group to add them.
  • Procedure risks are managed separately in Consent Forms β†’ Procedures, see Setting up or changing procedures.

Previewing the PDF

The preview shows the consent PDF using example patient details and updates as you edit. Use Preview as hospital to see the form for a particular hospital, and Download preview PDF or Open PDF in a new tab to check it in full.

Saving and publishing

  • Click Save draft to keep your changes without changing the live form.
  • Click Publish to make the changes live for the hospitals assigned to the template. The Published version number under the heading goes up each time you publish, so you can check your changes are live.

The Save draft and Publish buttons appear at both the top and the bottom of the builder.
