Open Registration Form → Edit Registration Form to open the Registration form builder. The builder lets your clinic choose which questions patients see, change the wording, and brand the form, without contacting Panacea AI.
The screen is split in two: the questions and settings are on the left, and a live Preview of the patient form is on the right.
Your changes won't reach patients until you click Publish. The Publish button is at the top right of the builder. Save draft only saves your work in progress; patients keep seeing the last published version until you publish.
Form settings
- Form title — the heading patients see at the top of the form.
- Practice logo — upload a PNG or SVG, at least 400 pixels wide. A transparent background works best. Use Remove logo to take it off.
- Accent colour — pick a colour or enter a hex code. The accent colour appears on the patient form (including the header bar) and on the registration PDF.
- Information for patients — a free-text block at the top of the form, for example your fees or what to bring to the appointment.
- Confirmation message — shown to the patient after they submit.
- Additional collection details (optional) — clinic-specific details that appear with the consent question about collecting and using health information.
- Allow save and resume — lets patients save a partly completed form and come back to it using a link. Set how many days the resume link stays valid.
Presets
Choose a starting point with the Preset buttons:
- General — the full registration form.
- Minimal — a shorter form with only the essential questions.
You can then adjust individual questions from either preset.
Turning questions and sections on or off
Questions are grouped into sections, such as Patient identity, Contact details, Next of kin, Medicare, cover & funding, Referral & referrer, How you found us, Your problem, Imaging scans, Medical history, Medications & allergies, Work, life & home, Anaesthetic & pre-admission, and Consents & agreements.
- Set each question to Off, Optional or Required.
- Some questions are Locked and always required because OrthoMate needs them to work: first name, last name, date of birth, mobile phone, email and the patient signature.
- Untick Section enabled to hide a whole section, and use the up and down arrows to change the order of sections.
- Edit the Question wording and Help text of any question to suit your clinic.
- Follow-up questions appear only when they apply. For example, the concession card number appears only when the patient answers Yes to having a concession card.
- Use Add question to add your own custom question to the form.
Custom consent clauses
Under Custom consent clauses, click Add clause to add your own consent or acknowledgement. Enter a Clause title and Clause wording, and tick Answer required (Agree or Disagree) if the patient must answer it. Click Remove clause to delete one.
Previewing the form and PDF
- Use Adult and Under 18 above the preview to see the form as each type of patient will see it. Guardian (consenting adult) questions only appear for patients under 18.
- Click Preview PDF to see the registration PDF your clinic will receive.
Saving and publishing
- Click Save draft to keep your changes without changing the live form.
- Click Publish (top right) to make the changes live. Patients who open the registration link after this see the new version. The Published version number under the heading goes up each time you publish, so you can check your changes are live.
Only non-sensitive, patient-facing copy belongs in the form. Web addresses pasted into consent and acknowledgement fields become clickable links on the public form.
The top of the registration form shows a For technical help phone number so patients can contact Panacea AI if they have trouble completing the form.
